Patient Enrollment

Verify the patient has a valid Kentucky Medical Cannabis Patient Card and must have a client record created by KMCP , in compliance with Kentucky Marijuana Laws.

Affidavit Submission

Complete, notarize, and upload the affidavit within your application to meet Kentucky Cannabis Patient Resource requirements.

Submit Caregiver Application

The submission of the Caregiver Application form as required by KMCP to assist in the care and management of medical cannabis for the patient, in accordance with Kentucky Marijuana Laws.

Background Check Process

Pass a fingerprint-based background check, processed by the Kentucky State Department of Health (KSDH) to ensure compliance with Kentucky Marijuana Laws.

Required Documents:

  • Caregiver Authorization/Patient Authorization Form
  • Proof of residency status (only attach your DMV Issued ID)
  • DMV Issued ID card (where asked)
  • Digital Photograph: Please see photo requirements in General Requirements” section of checklist (see below for information regarding photo requirements)

Requirements for Digital Photograph

  • Clear
  • Color photograph taken against a plain white or off-white background
  • Taken within the last six (6) months and must reflect current appearance
  • Applicant must be directly facing the camera with a neutral facial expression or natural smile, with both eyes open. Applicant’s face should be unobscured (no sunglasses, hats, masks, etc.)
  • The photograph may not be digitally enhanced, cropped, or altered in any way that changes the applicant’s appearance (i.e. no filters, no frames, no effects).
  • Photograph should be SQUARE and properly oriented – sideways or rectangular photos will be rejected.
  • This should not be a photograph of your driver’s license, passport, or any other previously issued ID

Registry ID card types

In Kentucky, individuals can apply for one of four types of registry ID cards:

In-state qualified
patient

Adults with a qualifying medical condition who have obtained a written certification from a registered medical cannabis practitioner can apply for a Kentucky Medical Marijuana Card, in accordance with Kentucky Marijuana Laws.

In-state minor qualified
patient

Individuals who are less than 18 years old with a qualifying medical condition who have received custodial parent or legal guardian consent and a written certification, may be eligible for a Kentucky Medical Marijuana Card under Kentucky Marijuana Laws.

Designated caregiver

Individuals who assist qualified patients with purchasing and/or administering medical cannabis may be designated as a caregiver under Kentucky Marijuana Laws.

Visiting qualified patient

Individuals with outof-state registry ID cards and documentation of a qualifying medical condition can be considered a visiting qualified patient, as per Kentucky Marijuana Laws.

Designated caregiver

Qualifications for
each registry ID card category

To qualify for a registry ID card, an applicant must meet the criteria below for their respective registry
ID card category:

In-state qualified patient:

  •  Resident of Kentucky
  • Must be a resident of Kentucky and have obtained a written certification from a Medical Marijuana Doctor in Kentucky with whom they have a bona fide practitioner-patient relationship, in accordance with Kentucky Marijuana Laws.

  • Must not have been convicted of a disqualifying felony offense under Kentucky Marijuana Laws.

  • Must be at least 18 years old to qualify for a medical cannabis program under Kentucky Marijuana Laws

In-state minor qualified patient:

  • Resident of Kentucky
  • Have obtained written certification from a medical cannabis practitioner with whom
    they have a bona fide practitioner-patient relationship
  • Have received custodial parent or legal guardian consent
  • Not been convicted of a disqualifying felony offense
  • Under 18 years old
  • Has documentation of diagnosis of a qualifying medical condition by a practitioner other than the medical cannabis practitioner who provided the written certification for their Kentucky Medical Marijuana Card.

Designated caregiver:

  • Resident of Kentucky
  • At least twenty-one (21) years old
  • Not been convicted of a disqualifying felony offense
  • Assist no more than three (3) registered patients
  • A custodial parent or legal guardian with responsibility for health care decisions for a minor qualified patient must serve as a minor’s designated caregiver

or

  • A guardian, limited guardian, conservator, or limited conservator of the qualified patient

Visiting qualified patient:

  • Non-resident of Kentucky or new resident of Kentucky who has resided in the Commonwealth for fewer than thirty (30) days
  • At least twenty-one (21) years old
  • Not been convicted of a disqualifying felony offense
  • Possesses a valid out-of-state registry ID card
  • Possesses documentation of having been diagnosed with a qualifying medical condition
    under KRS 218B.010(26)

Application fees

Initial application fees must be paid at the time an application is submitted by credit card or
ACH/eCheck. Application fees for each registry ID card type are outlined below
